Job Description:
Schedule: PRN or Part Time Open
Shift: PRN or Part Time Dayshift ***Orientation schedule is Monday-Friday dayshift for first 2 weeks.
Location: Covering Gordon County
Supervises and plans home health aide activities in the absence of other healthcare professionals. Demonstrates effective communication skills with all team members, patients, and families. Maintains clinical record information in accordance with established standards and within established time frames. Participates in orientation, in-service, and staff meetings as scheduled, and develops professional and clinical skills through attendance at staff development meetings, educational activities, and professional affiliations. Other duties as assigned. Participates in the team process through cooperative efforts with team members in developing the patient care plan and attending team conferences. Conducts home safety evaluations related to activities of daily living and communicates specific needs to the physician and appropriate team members. Instructs patients and families in the total occupational therapy program, including self-care and creative activities aimed at improving independence, physical, and mental functioning. Establishes household management routines to support patient independence. Displays flexibility in accepting assignment changes and patient care responsibilities, including participation in weekend coverage and call rotation. Performs comprehensive initial evaluations on all patients requiring occupational therapy. Establishes and implements a plan of care with goals based on the patient’s current level of functioning and potential for improvement, in conjunction with the patient, family, caregiver, and physician’s orders. Continuously evaluates the patient’s response to the occupational therapy program and modifies the care plan as needed.
Knowledge, Skills, and Abilities:
• A working knowledge of community resources and an ability to refer patients and families appropriately.
• Knowledge of Home Care Regulations and Third-Party Reimbursement as it impacts care delivery.
• Entry level competency of occupational therapy practice, theory, principle, and applications as defined in competency department policies.
• Basic computer skills and knowledge of Microsoft Office.
• Must be able to read, write, and speak conversational English.
Education:
• Master's [Preferred]
Field of Study:
• in Occupational Therapy
Work Experience:
• 1+ clinical experience as an occupational therapist in an acute or rehab setting [Required]
• 1+ of clinical experience as an occupational therapist in home health [Preferred]
Additional Information:
• Education requirements for this position are determined by the licensing or certification standards set forth by the relevant certifying body. Applicants must possess the level of education mandated for eligibility to obtain and maintain the required license or certification.
